- Backups that haven’t been tested in years
- Firewalls running software from a decade ago
Each of these can create an “interstate moment” — abrupt, costly, and entirely avoidable.
Why Smart People Fall Into This Trap
Intelligent leaders aren’t careless. They’re human. And humans are wired to misjudge risk in predictable ways:
- Familiar systems feel safe — even when they aren’t. Comfort becomes a substitute for evidence.
- No incidents equals perceived security. But “nothing happened” often just means, “nothing happened… yet.”
- Upgrades feel discretionary until disaster hits. Then they instantly become mandatory — and far more expensive.
- Low-probability, high-impact events are hard to emotionally visualize. So leaders under-prepare for them.
This is exactly why confirmation bias is so dangerous: it gives leaders the illusion of safety while vulnerabilities quietly grow beneath the surface.

- If our most critical system failed tonight, what would we wish we had done differently?
- Who is helping us see what we cannot see ourselves?
If even one of these questions stings, it’s a signal — not a problem.

Standing on the shoulder of the road, tightening the last lug nut, I wasn’t thinking about the metal that tore my tire. I was thinking about preparation — the quiet discipline that transforms a crisis into a manageable inconvenience.
I avoided disaster not because I was lucky. I avoided it because I was ready. Your organization deserves that same confidence. Not confidence in luck. Not confidence in the past. Confidence in preparation.
